When does your appeal window close?
The window itself is rarely the problem. The trigger is. A clock that runs from adjudication has often been running for a week before anyone looks at the remittance, and in nine states the statute says "the initial claim" without defining which event that is.
Two things this cannot do for you
It cannot tell you which event your state means by "the initial claim," because in nine states the statute genuinely does not say. Where that is the case your state page says so, and the safe reading is the earliest candidate date.
It also cannot tell you whether the claim is preempted. If it is a Medicare Part D claim, the state clock may not apply at all — the preemption note explains where that line sits.
