Filing a MAC appeal with CVS Caremark

Caremark treats MAC prices as confidential information under its Provider Manual, and the manual bars giving that information to a third party. That matters if you are thinking of handing your list to a vendor. It does not stop you requesting it for your own use.

Sign in at rxservices.cvscaremark.com, then Appeals Menu → Submit Appeals. One appeal per form, and the form carries a CAPTCHA. Portal access is restricted to one account per NCPDP number.

Before you file
The deadline that binds Caremark is usually your state's, not theirs — and in most states the clock starts earlier than people assume. Work out your date, then check what your state requires them to disclose when they deny it.
