Filing a MAC appeal with Express Scripts
The fax step is the problem. The portal alone does not complete the appeal, and it is easy to think you have filed when you have not.

Three steps in the PRC portal at prc.express-scripts.com, which then tells you to print a fax cover sheet and fax the invoice separately. There is also a MAC department mailbox.

- Express Scripts does not publish its own filing deadline. Treat your state statute as the operative clock.

Before you file
The deadline that binds Express Scripts is usually your state's, not theirs — and in most states the clock starts earlier than people assume. Work out your date, then check what your state requires them to disclose when they deny it.
