Who actually answers
Forty jurisdictions impose duties on PBMs to disclose MAC pricing and to answer appeals within a stated number of days. Nobody publishes whether those duties are met. Not the states, not the PBMs, not the trade press.
So this is the record, built from what owners report. It will be thin for a while and the sample size sits next to every number, because a handful of reports is a handful of reports and presenting it as a finding would make this site exactly the kind of source it was built to correct.
Everything else on this site is free and open and stays that way — the statutes, the request letter, the deadline calculator, the PBM contacts. So are the response rates below. The one thing you have to add a report to read is what other pharmacies wrote about how it actually went, because that part only exists if people put it there.
What they say they will do
Half of the eight endpoints publish a turnaround they hold themselves to. The other half publish nothing at all, which is its own answer. This column is from each PBM's own guide — the right-hand column is the one only pharmacies can fill in.

Turnarounds as published by each PBM in its own appeal guide. Where a PBM publishes nothing, your state statute is the only clock that binds it — which your state page gives you.

Every figure would carry its report count, and nothing publishes below five reports for a PBM. That threshold is the point: a response rate built on three reports is not a finding.
